An independent body composed of tech experts, ethicists, and legal professionals that investigates instances of widespread AI failure or harm, determining causes and assigning accountability, ultimately publishing findings for public consumption as well as recommending policy changes to prevent further algorithmic failure. Their purpose would be to get to the truth behind the algorithms for public record.
